who wants just a lawn when you can have no mow areas and impact the noise and air pollution and have a little oasis away from the rat race but you can use a cultipacker but you dont want it to packed down cause having areation is a good thing have you seen those rolly things that you fill up with water
The only real way to do that is to roll it. That will get rid of the frost heaves. After rolling it, though, aerate it so that the soil isn't too compact.
To repair the patchy grass areas, check the soil and make sure it's got the proper pH (get a kit at the home store) and then overseed with grass seed for the same type of grass you already have.

Those are two very different questions.
The best way would be to rent a rototiller, till the entire thing than rake and roll it level. Then plant seed or sod.
The easiest way is to hire a landscape contractor to do it for you
