All of the suggestions given already are good. I live in damp Seattle where weed seeds germinate on almost any surface. In the spring, I remove winter sprouted weeds and grass from my patio stones by hand and then lay fine sand over the patio and sweep it into the spaces between the stones. I do this every spring, so the spaces between the stones are mostly sand, and if any weeds do manage to sprout there during the year, they are easy to pull up and be rid of. It works for me.
Vinegar, spray @ least 2-3 times a week, full strength.
