What's the best way to get rid of couch grass in lawns?

You should use a selective weedkiller containing the active ingredient 2,4- dichlorophenoxyacetic acid.
Selective weedkillers are usually applied to the lawn as part of a weed and feed application, but can be applied separately. Their action is usually hormonal affecting growth rates, mono- and dicotyledonous plants(narrow and broad leaved respectively) are effected differently. This works due to the different growth habits of the plants, monocotyledon leaves grow continuously from the base, eg, grass, and are less susceptible to the chemicals. They are usually used in combinations of two or three to over come resistant weeds. Do not apply to lawns less than 6 months old, either seeded or turfed.

Do not set your mower blades too low as couch grass likes to be cut short. Cut the lawn more frequently to keep it looking tidy.Try feeding the lawn with a high nitrogen based feed. The other grasses should then smother the couch.
You can pull it out by hand, roots and all.
There used to be a couch grass treatment on the market, but it was withdrawn in the UK, because of its toxicity. However, you can try killing it with a powerful weedkiller, like Glyphosate, but be very careful in its handling. Use a brush and apply a small amount to each plant. Keep pets and children away fro the area for a while.
To remove couch grass by hand is almost impossible, in that the underground rhizome is often many feet below the soil surface.
