Answers:
Any glass shop that sells shower doors will be able to get you tempered glass. You will have to have the hole made in the glass prior to tempering Once its tempered you can't drill or cut it. So you will have to give them a diagram of what you want. Get out the phone book and start calling bigger glass shops. It's not going to be cheap though.

I find the size you are asking for incredible, especially if you are trying to save money. 12 feet of tempered glass is going to be hard to get and very awkward to handle. And you are putting it in front of tile which is normally an easy to clean substance. I don't know what you expect to "make it myself" but you certainly can't do the tempering of the glass although you probably, with great care, can screw mount it.
