Turn the water supply off, drain all water in the toilet, disconnect the water supply, disconnect the tank and disconnect from the floor. Once out of the way make sure you clean the area really well, place down a new wax ring, place the new toilet on top of the wax ring, tighten the toilet to the floor (bolts which go on the two studs sticking up from the floor), install the tank (with all of the goodies inside ready to use), tighten the tank down, reconnect the water supply and turn the water supply on again. Check for leaks and you're done.
With a lot of work, Its not something I would recomend trying to do without a plumber.

Linda D pretty much has it covered. The only things I would add are after you put the new bowl on the wax ring-- sit on the bowl to squish out the wax ring also replace the two bolts that hold the toilet to the floor before you set the bowl since they are probably corroded pretty badly. Lastly, don't over tighten the bowl to the floor so you don't crack the porcelain.

The procedure is quite simple, but please note that a toilet can weigh about 100 pounds.
1st: Turn turn water off using shut off valve located to the bottom left of tank on the wall or floor. Turn valve completely to the right.
Next, flush toilet one final time to remove all water form the tank.
Then disconnect supply line from the tank located underneath the left side of tank.
Then locate the nuts at the bottom of bowl and remove. (Depending on how long the toilet has been there, the bolts may be rusty and need a little WD-40. If that doesn't work you will have to cut the bolts just below the nuts using a hack saw.)
Next lift toilet from floor.
Read instruction from new toilet on how to assemble if purchased in parts.
Once toilet is assembled make sure t-bolts (bolts used to secure toilet to the floor) are positioned in the 3 o'clock and 9 o'clock positions of the flange. Then place wax bowl ring on bottom of toilet making sure it is secured tightly but be sure not too tightly for wax to spread.
Now your ready to reinstall toilet. Reverse procedure starting with tightening the nuts to secure toilet to floor and supply line to tank.
Turn water back on and check for leaks!
