When installing carpet, how do you get the tack strip to not poke through the carpet?

When a professional stretches the carpet in They rub down (bend over) the tacks with a "Stair" tool (like an all ,wide blade chisel) Sometimes a few are missed.

Like another`s answers, simply hammer the offending tacks down Hard. If you still have one or 3 that is a problem take pliers. or vise grips and pull them out putting pressure on both sides of the tack so the whole strip does`nt come up.

Underlayment will make the carpet and pad too high for the tacks to get a good bite on the carpet. Near impossible to get a Good stretch on it.

Never put pad over the tack strip for the same reason above.

Your supposed to have padding under it.and have the points of the strip facing the wall.Then use your knee-kicker to stretch the carpet and pad up and over the strips.
put down an underlay, just make sure it isn't thick enough to stop the tack strip gripping the carpet itself.
The tack strip is typically so close to the wall it doesn't matter. However, if you have a spot that it's an issue, just tap the section with a hammer to blunt the points.
