Ah, an easy question to answer - LOL!

Most of the larger craft stores carry foam rubber sheets up to 4-5 inches thick, but sofa and chair cushions are usually much thicker.

When I replaced the foam in my cushions, I got the foam from a local upholstery shop (they're listed in the yellow pages) at a very reasonable price, too.

*** Foam rubber is a real pain in the butt to cut with a knife, razor blade or scissors. Invest in a cheap electric carving knife, mark cutting lines on the foam for your cushions and away you go... You'll have perfectly cut foam pads that won't look like they were gnawed on by a giant rat!
I got my foam from a glass company that shipped thermoform glass in it originally.They gave it to me for free,and in good shape.

I bought a foam mattress (twin size) and just this morning cut it to fit my sofa, I used an electric carving knife and it works great. I was able to cut it to fit exactly to the size of the sofa.
Joanne Fabrics, as said before, is a good place to start... they have sheets of foam rubber ranging from .5" to 6" thick.

I've replaced a 6" sofa cushion w/ two 3" thick pieces with a sheet of cardboard between the two to "firm it up" and then wrapped twice in polyester fiber (also available in fabric & craft stores) fill to soften the foam edges in its cover.

Cutting foam rubber is easiest with an electric knife, but a serrated bread knife will also work well. Don't forget to add 1/4" - 1/2" to the overall dimensions of the foam to ensure it expands well and totally fills the cushion cover.

To re-stuff the cushion cover with the new foam rubber... seal the foam rubber (preferably wrapped in batting) inside a plastic bag (a large, black trash bag or two works well) and suck out the air in the bag (use your vacuum cleaner hose) and then quickly insert the new cushion into the cover... slide your hand down the inside of the cover to its bottom and tear the plastic bag enough to remove it without removing the cushion.
