Find out what brand of toilet you have. Go to local hardware store or plumbing supply store and get compatible flush valve kit and fill valve kit. They should come with instructions.

Be sure to turn off water source, get as much water out of the tank as possible (sponges come in handy for this). Put a small bucket under the fill valve when you take it out.

Good Luck!

It is very easy. You can buy the parts at any home improvement store. The instructions are on the box. Also you can get books on plumbing from the library.
I had to do this very same thing at my mothers house last year.
Her house was built in 1958 & still had the original toilet w/plumbing parts.
As the others have stated, go to the local hardware store & get a compatible flush valve & flapper valve. Installation instructions provided
Deoending on what type of toilet it is can be pretty simply get to a local hardware store (not a big box) like Ace or True Value, the sales people are very helpful and can help answer any other questions, if you get right parts pretty simple to do
Take a picture of the inside of your toilet tank, print it, and take it to a hardware store. They should be able to tell you what you need to buy, and the instructions will be on the packaging.
