Looks to me like a simple screw driver. Just that the handle is shorter and also made of iron.

Looks like some type of awl or punch to me.
Could it possibly be an unfinished stock for a mandrel used in pressing fittings on hoses? See the link below for pictures of what a finished one looks like. I've used these before and they vary in the type of construction. Some have the threads to screw the fitting on and some are smooth.

I'm 100% convinced my above answer is wrong. Do you have any more information on the tool? Is it something you own? Or where it was bought or found? I noticed the patent pending on the handle and searched for a few hours in various patent websites. The hardest thing is not knowing what to search for. Came across a few bone surgical tools and one for dental surgery that come close but nothing like the picture shows. My curiosity is way up on this one. I'll keep on looking.
