It depends what's wrong with them and what type you're talking about (steel columns or anchors for wood nailer).

For steel columns, if the bolts are not set in the right place in the concrete, you can either blow the holes in the columns larger or cut them off and epoxy new ones in the right place. If the bolts need to move more than half an inch, I usually cut them off and drill for new ones. If it's less than half an inch, I cut the holes in the base plates larger with an oxy-aceteline torch. I use larger washers when I enlarge the holes. If they are just crooked and not lining up with the holes in the baseplate, I put a nut on the top and beat them into alignment with a sledgehammer.

If you're asking how to secure (fix) them, you can either wet set them or drill and epoxy. Wet setting is when you make a template out of plywood with holes that mimic the bolt pattern. You put a nut on top of the plywood and one on the bottom to secure the bolts and set them in the wet concrete. You can align them ahead of time by suspending them exactly where they will go before the concrete arrives, or you can "shoot" them in after pouring by using a transit and/or strings.

If you're talking about anchor bolts for the nailer on top of a block or concrete foundation wall, use J bolts and set them any way you like. You'll have to custom drill each hole on the nailer anyway, so sterssing over placement is a waste of time. You just set the nailer on top of the wall and transfer the locations of the bolts, then measure from the face of the wall to the center of the bolts. This will give you the location to drill for each bolt.
