You can go out and buy a self leveler like custom building products levellite or level quik. This can get expensive. I own a tile company and what we do is use the thinset to build up any individual tiles that need it. Basically just butter the backs of any tiles that need to be raised a bit. If you've never done tile work you may want to go with the first option or hire yourself someone that knows what they're doing because trying to get the tile level on concrete can be a b*tch.

This depends on how bad the concrete is. If there is just a few areas where the concrete sticks up then a hammer and chisel should do the job, but if its really bad then you might need to level it and you don't have to use anything expensive, you could just use sand and cement glue over the top.

they make a floor sealer that is self leveling. most home improvement stores sell it. It not real expensive it comes either in small or larger quantities. you just mix it with water and pour it on the floor. I used it on a mobile home floor to put tile on the bathroom. it worked really well.
