Kill the groundhog.
The only way to fix it for good is to dig up the tunnel and fill it with rock and compact it.
Where the driveway is caving in, cut away the pavement about 1 foot on each side. You rent a special saw for this. Remove the cracked pavement. Then dig up the rock underneath setting it on one side of your trench. When you get to the soil dig down to the bottom of what was your tunnel and fill it back up with soil, compacting it as you go. Then put back the rock, compact the rock and add until you have filled your trench to about 3-4" below the surface of your driveway. Then fill it with a few bags of cold patch or have a paving company fix it for you.
First thing you need to do is get rid of the groundhog. If you have a cat, then try putting some urine soaked litter into the hole. Since a cat is a predator that will occasionally get them to leave. Put grass or hay in front of the hole so you'll know if the groundhog has left. Once it's gone, the easiest way to fill in the hole may be to pump in concrete as far as you can. You don't say if the driveway is paved or not. If it's not, then you can always just refill the caved in spot with whatever is already there as it caves in, but make sure to at least fill the entrance tightly with dirt so you don't have water draining under the driveway and causing erosion as well.

Get a culvert big enough to hold up the driveway, start digging on one side of it and shove the culvert in it. Then the groundhog will have a right of way and your car has a driveway.
