Although not recommended, it can be and has been done. As suggested, you must sand the old formica to scuff the surface. Anytime we have done this, we have used 60 grit sandpaper in an orbital sander. The more of the old surface you "open-up" with the sander, the better chance of the new formica bonding.
We have made it a habit to always apply two coats of contact cement to both surfaces to insure that there is glue enough to bond. I believe only one coat is recommended normally, but our experience has made us apply two.
If you were to go over new wood - particle board or other, then you might need more than two coats. New wood will "suck in" too much of the glue so that there is not an even application. Also, there is always some area that you never get enough glue on with one coat - that is always the area that comes back to haunt you by coming loose first.
Once the surface is prepped, then you are ready to install your new formica. As with almost any project where you are bonding two things together - paint, glue, tile and others, surface preparation is the most important step. Poor prep and no matter how good of installation there is, in time the lack of prep-work will show.

it very possibly could work,but you will want to scuff up the surface with a 120 grit sand paper first to give the glue something to stick to.
I suppose you could, but I wouldn't recommend it. You'll end up with two layers instead of one; which can increase the chances of delamination. Most countertops with a laminate such as formica, are simply particle board underneath and that stuff is cheap. I would just replace the countertop and have everything new.
