Futons are fairly comfortable for sleeping IF you get a good mattress. It'll cost a little more, but it's definitely worth getting the extra thick mattress if you are going to sleep on it. The store I bought mine from had some covers in the store and a catalog with a lot of covers you could choose from and they would order for you.
Yes, you can get a futon cover. Try the local Bed Bath and Beyond or something along that line. I saw one in an ad last week. They have several sizes, I believe. Price likely depends on the size of your futon. If you're going to sleep on a futon permanently, make sure you get a good futon. We have a metal one that works great; I've slept on the ones with wooden slats, and when the futon pad gets worn down, you can feel the wood on your back all night. Makes for a long night!
I don't really know about the custom cover, but I know that you can buy a variety of futon covers in various colours, patterns etc. But here's a couple of links:

If you google "Futon covers" you'll find loads more (though the tonicliving.com is my personal fave. I <3 retro.)
As for the comfort, I'd say it's comfortable enough. I haven't spent all that much time on them, but I've had friends that used them as beds and they didn't have any complains. It's probably not going to be as comfortable as a bed would be, but it sure as heck would beat a floor, air bed or a plain ol' mattress. You should still be able to a good nights sleep.
