It depends on the type of paint that is on the tile. A good place to start would be with mineral spirits (paint thinner). I would start with a 50/50 solution in water. If that makes some improvement and you are confident you are not damaging the finish on the tile, you can increase the concentration. Again, depending on the tile, if the mineral spirits does not do the trick, you can step up to laquer thinner. Follow the same procedure as with the mineral spirits.

If all else fails you can also try brake parts cleaner (you can get it at an auto parts store). Another idea is using a heat gun (a hair dryer can be substituted, but it wont be as hot) and a plastic putty knife to heat and scrape the paint. If the grout is painted, you will probably need to remove the grout and re-grout the tile. Removing the grout is easist with a rotary tool like a rotozip or dremel tool.

if it's acrylic/latex paint... try a putty knife or very very hot towels then the putty knife.

if it's oil based, try mineral spirits
If it's just a few spots, I use my fingernail. Do not use anything that can scratch the tile.
Try using something like nitromorse paint remover. You can buy it in a small tin for not a lot of money. Apply with a brush to the affected area, then clean off with a light scourer and sponge with warm water. Repeat the process as necessary. Make sure to neutralize with water to finish
Use a Dobie pad. It is a nylon scrubber with a sponge inside.
You can remove any kind of paint with the pad.You may have use more elbow grease with a non-latex paint. It will not mar the tile.

Buy in the grocery store.
If it is a tile that don't scratch easily, a putty knife or even better the blade out of a Utility knife. (be careful) A tile that scratches easily, and hopefully a water-based paint Moisture first then you can probably just wipe it away after a few minutes. If it is an oil-based paint don't put the water to it.
Best chances are to let it dry(the paint ) if it has been there for a bit. "It will smear" may be able to just flick-off with your
finger-nail....if that don't work, paint-thinner- mineral spirits.
