How can dried tile grout that has been left on the tiles and floor be removed?

The tile section of most home improvement stores sell haze remover. The active ingredient is phosphoric acid so be sure to wear protective clothing and plenty of ventilation. You'll probably have to do it several times to get the thicker stuff off but if you keep at it, you'll get it.

Plastic scrapers from the paint dept. work well at getting the big stuff without damaging the tiles much.

Haze can be removed from tile with Muriatic Acid.
Chunks of grout need to be chiseled off. Pain in the
butt. If it's fresh, you might be able to scrape it off
with a putty knife.
you will need acid for the floor probably

as far as the tiles you may consider just buying replacement tiles.
I used a steam cleaner and paint scraper .. it worked but what a pain.. need to get it before it dries really.
