Mild acid cleaners, such as those with hydrochloric, oxalic or phosphoric acids, will remove rust (with some rubbing from a _mildly_ abrasive pad). Stronger chemicals such as hydrofluoric acid will remove rust rapidly but _also_ destroy the smooth finish on the porcelain, so that it will lose it's shine, soak up stains more quickly, and possibly might even be damaged structurally.
To prevent rust in your plumbing and fixtures, the most effective way is water treatment, such as by ion-exchange resins. It is also possible to add small amounts of chemicals that neutralize the water, preventing pipe corrosion which is deposited as stains in fixtures; but this is best done at the town water supply, since it requires careful monitoring and testing. In Spencer, MA, there was an incident involving some (luckily mild) burns from this caustic treatment.
If you have rust in the toilet it may be in all your water -do you have steel pipes? I'd be more worried about the laundry. You can treat the toilet tank if that's the only problem-there may be some old rustly parts in there that can easily be replaced (by a plumber if you don't want to try).
I'm not positive about rust but I use Sno-Bowl whenever bleach isn't cutting it, and my mom thought that was funny cause she said when my grandma moved to a new house the toilets were nasty and she cleaned them with sno-bowl and they looked great. So it it was neglected probably there was rust in it and Sno-bowl got it out. To keep it out, just clean it regularly. If it doesn't get used a lot, flush it every few days or so so the water doesn't sit too long.

Try a product called Iron-Out. The same product can be used in water softeners and should work well for you.
