Have Rosie O'Donnel camp out at your home for a month and tell her that is the only toilet she can use.
Just like the Grand Canyon was carved out by the Colorado River, your stains will be eliminated by that pig's erosion effect. The calcium and lime stains will be removed, replaced by some stinking moldy looking bacterial oozy scum stains. The Porcelain may be cracked and the wax ring compressed to dysfunctional status, and the fire alarm may go off from the stink, but you will have your lime and calcium removed.
LimeAway is a very good cleanser to try, good luck.
A product called CLR is great you can buy it at wal-mart. It is made just for that stuff it means calcium lime and rust.
A pumice stone works great on toilet bowl rings.
Use Lime-A-Way and baking soda and let it sit overnight.
Then after toilet is clean, use a pumic stone to scrub the minerals out. You should get some kind of water filtering system for your residence if you can. Hard water wreaks havoc on appliances and fixtures.

Lysol toilet bowl cleaner..=)
Turn off the water which fills the bowl and VERY CAREFULLY pour small amounts of diluted muriatic acid over the stain. Make sure ventilation in the room is good, using open windows or fans and wear a mask and gloves. IMPERATIVE, wear mask and gloves.
