Though I read this several years ago and can't remember my source, I've got this to work on several older stains (it's ALWAYS worked on new stains) -- but I can't promise that it'll get the stain out completely.
Soak the pillowcase in cold water -- as cold as you can get it -- and rub in some Head & Shoulders. If you have an old toothbrush handy, it really helps, but you don't NEED one. Then wash it as normal. You may have to do this a couple of times. There's an enzyme in the shampoo that breaks down the protein in the blood, and this is supposed to loosen it from the fibres in the fabric.

use peroxide and water then rub gently with dawn dish soap and water... let air dry
I've used Folex, this stuff works great (not just for carpets). Try it, it's very inexpensive. Good luck!
Remove Old Blood Stains:
To clean blood stains from clothes reactivate the blood by soaking in cold water for a few minutes. Ring out the water well then pour Hydrogen Peroxide directly on the stain and allow to foam, before washing. The peroxide will break down the protein in the blood and release the stain.
In most cases this will do the trick, however if the blood has bonded with the fabric you may have a harder time, if this is the case you should try Sugar Paste: Sugar Paste, made of sugar and water, and allowed to sit for one hour, you will not need to scrub, however if possible it will help to break down the stain!!

Hydrogen Peroxide. My husband uses it all the time on his nurses scrubs.
pretreat with peroxide or oxyclean powder with a little water, make a paste, let set for a couple of minutes, rub and rinse to see if it's gone, if you can still see it, repeat then launder as usual.
