1. Get it professionally cleaned. This may cost $70-$110 or so and will take a few days, a reputable dry cleaner may arrange this.
2 Obtain a bottle of liquid wax polish in an appropriate colour. Go over any areas where the colour has begun to wear off. This stuff contains a lot of dye and will re-colour the worn areas. This can be a bit messy so take care where you do it. Hang the jacket on a good hanger and leave it 24 hours for the stuff to sink in. Black is obviously the easiest, if you have some other colour you might have to skip this step.
3. This might be enough, but if the leather is getting old and stiff take the jacket to a saddler and ask them for a suitable product. One of the most favoured is a combination of mink oil and beeswax. I have used a leather product containing mostly beeswax and lanolin and found that OK, I mainly use it on leather car seats. Wipe on generously, paying particular attention to wear points like the zip opening, pocket edges, seams etc. Allow the stuff to sink in overnight or 24 hours with the jacket in a warm, not hot place. This treatment is recommended at least once a year. A good L-J can last 50 years.
4. Rub off any excess stuff and wear.
5. Keep the jacket on a good wide hanger in a wardrobe/closet.

You can go to a leather store and purchase a product to rub into the leather that helps keep it soft, and not get brittle. This tends to darken the leather a bit as well, which will hide small surface scratches.
