The holes my mom keeps finding in only her t-shirts even appear after only wearing one time after bought.?

Check the closet and were she is putting the close it may be infested with a pest that like eating cotton.
Mice will do this to just about anything
It's probably either mice or moths.
You can get some humane traps for mice at the local hardware store, I believe.
For the moths, put some moths balls in and around where your mom keeps her shirts.

If it is not a pest issue (as already covered) it sould be a 'wear' issue.

Thin materials could be getting 'poked' and holes started during the course of the daily use she is putting them through.

Perhaps wire piping in upholstery of a chair or couch that only she sits in is 'catching' the shirt and causing the trouble. Maybe the shirt is very thin and even things like the little quills of a down jacket (which often poke out inside the jacket) are pushing holes into the weave of the fabric.

Welding, soldering, or smoking are common sources for little flash burns that can burn holes into materials.

If no one else is experienceing the problem, I would look closer for an environmental cause (where mom goes and/or what she does that no one else does) then suspect a household pest that would probably not be restricted to only one household member.
