If you can find pipe cleaners, they work great for straws --even those curly ones that little kids just love to use and "bendy" straws--some times a double ended baby bottle brush has a small enough end to run down a straight straw as well. Running hot water down it after every use will help keep them clean as will a good soaking in hot dish water or a bleach/water solution. Remember to wash/rinse all straws that aren't disposable immediately after use to prevent this problem in the future

Soak it in some bleach and water for a while. Make sure to rinse very well afterwards.
If you mean the single use ones in restaurants...they are single use for a reason.
If youre talking about the re-usable ones that come with some sports bottles...because you don't wash it properly, or even promptly after using it.
At this point, if it is replaceable, do so. If it isn't, discard it and recycle the bottle it belonged to.
