Use laundry detergent and a toothbrush and scrub a lot! Once they go through the drier they are pretty much permanent. Get as much out as possible before you wash in the machine.
i usually pour a cup of white vinegar in my really bad laundry. some times i use a can of cocacola.
Try SoftSoap, its an antibacterial hand soap and I swear it works wonders. We got a black ink stain out of our antique rocking chair seat that had been sitting for a few months. Just scrub clothing with some softsoap and water and it should come out. But I'm not 100% sure if it'll work on grass.

I have had good luck with Shout Stain Stick. I think it works the best for grass stain as well as most others.
