Answers:
I pretreat all my stains with oxi clean. If the colthes are white then I also use bleach on them.

Try pouring a little bit of bleach in the wash along with your detergent.
Add Chlorox bleach to the wash water.
If they are light colored, try bleach with your next wash. If that doesn't work, buy new ones.
hmmm...not sure. if you can't get them out...buy new pillow cases and wash them more often. like once a week.
Simple Green. Works on anything. If simple green does not work-buy new ones.

I cant believe i answered a cleaning question.
Get some new ones. They don't cost that much. Probably less than all the chemicals your going to try.
Now there are washing powders based on enzymes. They will clear such stains.I donot know the brand name popular in your area. They work under low temperature. You must not use it for hand wash, may be allergic or use gloves or washing machine.Surf e-shine is an enzyme based washing powder.
