What's the best way to rid a shirt of a permanent sweat odor that doesn't come out in the wash?

I would probably get rid of the shirt. Better that than feeling and making others uncomfortable.

Before thinking of throwing it away, try this one . Perspiration stains: Soak in White vinegar ( 2 cups white vinegar to 8 cups of water) It doesn't say for how long I would give it at least 2 to 3 hours, and then rinse.
Add a cup of baking soda to your wash cycle. It will deoderize along with your detergent. And if the fabric will allow, use super hot water.
My mom used to have this issue with my father's sweat-stained tee shirts.

Buy a box of washing powder called 20 MULE TEAM BORAX. It's in a green box and you can find it just about anywhere. Mom gets it at Wal-Mart.

Wash the stained shirts by themselves in hot water and add a cup of this BORAX. It will completely remove the old stuck deodorant that causes the stains and odors! (^_^)
It depends on what you've got handiest around the house.

White Vinegar removes odors...
I use white vinegar to get smells out of laundered clothing. I use about a cup, added to the wash load, with the normal amount of detergent.

Table salt works to remove smells too. Add 4 cups of table salt along with your detergent and fill the machine. (no clothes yet) Let it agitate for a minute or so, then stop the machine, add your clothes and let it agitate for another minute to make sure the shirt / clothing is saturated. .Then turn the washer off and let soak overnight. Next morning, turn the washer back on and let it finish the regular cycle.

20 Mule Team Borax is excellent as well. Just follow the instructions of the box.

Liquid dish soap is good, because the sweat stains and smells are body-oil based, so dish detergent can cut right through the oils, leaving your shirt smelling fresh.
