Some tiles are loose in the shower? is this because of mould or faulty construction?

Question:The house was built in 1994-95. It's only a few tiles that have come a bit loose just above the bathtub. I just want to know if this is because of neglect (whether mould actually helps deteriorate the tile adhesive) or whether it's faulty construction (grout is not waterproof and if not applied properly it will aggravate the problem).

Answers:
I suspect it is more faulty construction, mold probably doesn't help, but I doubt it is the whole cause. does the tile have "water resistant" drywall behind it or cement backer board? the "resistant" drywall is nowhere near resistant enough for a tile installation.

It could be the water and the humidity in the bathroom when taking a bath or shower. My grandfather did tile work and laid carpet. You need lots of air circulating in the bathroom when taking a bath. also When done taking a bath make sure the tile is dry that helps too. The house I live in now my grandfather did the tile work in my bathrooms and they have not come off yet if has been over 20 years. My grandfather always said to maintain a shower or tile in bathrooms make sure they are wiped dry.
You are so right on the grout you may have to get waterproof grout and redo the tile.
