Well you can try and steam clean it ,I think the problem might have gone to far for this, so the only other thing I can suggest is to replace that piece of carpet/tufts If you have a spare piece of carpet ,or take a piece from a place where it will not show,Remove this piece of carpet with a sharp trimming knife or safety razor, to expose the backing beneath. Treat this first with a proprietary fungicide. Once this has been done, dab the bottom of the hole and the ends of the piece / tufts with a suitable latex adhesive, then insert the carpet / tufts into the hole. Leave to dry , then trim if necessary and brush with the pile
Replace carpet.
It might be too far gone at this point. you can try the rug doctor again, but add bleach to the shampoo mixture. Even if it wont remove the mold stain, it will kill the mold and keep it from spreading. If the stain wont come out, put a throw rug over it.
