When should a dish sponge be thrown away?

Sponges will last as long as you take care of them. I put mine either in the washing machine, along with the towels, or the dishwasher, on the top rack. Granted, they won't last forever. I usually replace mine every 3 months or so.
Dip it into disinfectant after each batch of dishes and you should be able to use it till it's worn out

I usually throw them away when they start falling apart. I wash my sponges in the washing machine so they get clean. Don't put them in the dryer though or they will get dried out. You can still use them if they go through the dryer but I try not to put them in the dryer.
Depends on what you soak up with it. Lot of meat and chicken juice, I'd throw it away after a week or two.
If it's just used to clean dishes and glasses and plates, it can last longer, especially if you're using an anti-bacterial dish wash.
Usually I just judge it by the funk. If it stinks, toss it.
Also, if your worried about it, you can microwave it for a minute on high power to kill the bacteria in it.

Each time you run your dish washer throw your sponge in the machine, the water in a dishwasher gets hotter than tap water. It will kill many germs, also you can throw your sponge in with your load of whites, In general if you do those things you can keep a sponge for up to 6 weeks. :)
I throw them away when they dry and they begin to curl up too much and have any odor.
After two weeks. Make sure you disinfect with bleach every couple of days.
i replace mine once a week. I dont want to take the chance of anything happening.
